HealthSpan
Reimagining whole-person wellness as a community goal
HealthSpan is an innovative community health collaborative in Nevada County, spearheaded by Sierra Nevada Memorial Hospital and community leadership. Recognizing that traditional healthcare focuses on treating illness after it occurs, HealthSpan shifts the paradigm to address the social and lifestyle determinants of health that impact well-being and longevity. Through cross-sector partnerships, HealthSpan aims to strengthen systems, programs, and policies that promote physical, social, and spiritual well-being for all residents. Its core mission is to reduce chronic disease, alleviate strain on the healthcare system, and enable every individual in Nevada County to live longer, healthier, and more meaningful lives by fostering environments that support purpose, connection, nourishment, and movement, ultimately serving as a prototype for communities nationwide.
