Social Outreach Program

 

The Social Outreach Program is a Prevention and Early Intervention Program supported by Nevada County Department of Health & Human Services and the Mental Health Services Act.

The goal of this program is to support and promote the mental health of isolated seniors and adults with disabilities.

The Social Outreach Program offers help and support at no cost to those having a hard time reaching out.

The program can come to your home at your convenience, provide a confidential assessment, and can offer referrals, resource information, or emotional support to maintain independent living.
