Sierra Nevada Memorial Hospital Foundation is proud to be the fiscal sponsor for HealthSpan Nevada County. HealthSpan is an innovative community health collaborative in Nevada County, spearheaded by Sierra Nevada Memorial Hospital, the County of Nevada, local nonprofits, and community leadership. 

HealthSpan Nevada County brings people and partners together to make healthy living easier in everyday life so that more community members can experience lasting well-being. 

Our work strengthens and connects the programs already serving the community while advancing the policies, systems, and environments that shape long-term health. Our work centers on four focus areas: nourishment, movement, social connection, and supporting youth and families to create a healthier Nevada County for every generation.

In 2026 and 2027, HealthSpan will launch with the following projects:

  • Health in All Policies helps bring a health lens into local decision-making, planning, and policy conversations. Through this work, HealthSpan will support partners and local leaders in considering how local decisions around transportation, housing, food access, land use, parks, economic development, and community services (etc.) shape everyday health and well-being.

  • Social Care Coordination focuses on making it easier for community members to connect with the services and supports they need. Through this work, HealthSpan will bring partners together to design a more coordinated referral system, strengthen communication across organizations, and support a more connected network of care across Nevada County.

Meet our HealthSpan Director:

HealthSpan Executive Director

Kristal Caballero, MPH, is a bilingual community health leader with over a decade of experience spanning nonprofit organizations, state and local government, and cross-sector community health initiatives. Her work is grounded in advancing health equity and designing community health systems and policies that make well-being more accessible across the lifespan.

Kristal holds a Master of Public Health from the University of California, Berkeley. She is a proud mother of two young children and resident of Nevada County, where she and her family enjoy the region's abundant natural beauty, arts, and culture.
